Bias Incident Report Form

The University of Evansville is committed to making sure all students and employees have a space they are able to report incidents of bias or hate. This form can be filled out when someone is either a target of or witness to a bias-related incident. Individuals are encouraged to report this incident by completing the Bias Incident Report Form available below, or contacting the University of Evansville Center for Inclusive Excellence office at 812-488-5261, or by email at cs175@cp55586.com.

The Center for Inclusive Excellence will address reported incidents based on the information provided. Therefore, please provide as much detail as possible about the incident.

Reports may be submitted anonymously. However, this may limit the Center for Inclusive Excellence’s ability to address the situation. If you share your name or contact information, privacy will be the top priority. However, some information may have to be shared with appropriate campus officials if there is a safety or criminal concern.
